Terms of service.

 

The following Terms of Trade apply to services provided by Ashburton Health First to its patients.  By receiving service, you hereby agree to the Terms and Conditions of Trade as stated:

 

  1. Our GP & Nurse consultation fees are displayed in the reception area and on our website. Copies are also available; if you would like one please ask at reception. Our fees are made up from the following:

    • Consultation time

    • Complexity

    • Eligible funding

    • Cost of running a medical practice

  2. Consults that require additional services and/or extended time will incur additional fees such as, but not limited to, ECG, smears, referral letters, and repeat prescriptions

  3. Prices include GST unless otherwise stated.

  4. Prices quoted for services may be adjusted from time to time, and the customer hereby agrees to pay any such adjusted price, e.g. in instances where cost of goods increases, government surcharges increases, errors or omissions by Ashburton Health First or its representatives.

  5. If you are not enrolled with our practice but would like to see a doctor the consultation fee is higher. This is called a casual consultation.

  6. All services are to be paid for on the date of service. There will be an additional charge of $5 if not paid on the day.

  7. We accept Cash, Eftpos or Direct Debit. Credit cards are NOT an accepted.

  8. If you have financial difficulties we can set up a direct debit or complete a WINZ request if receiving a benefit.

  9. Ashburton Health First may withhold further provision of service where there is any outstanding amount due, unless presenting with a Medical Emergency.

  10. Travel vaccines must be paid for when the appointment is made as they have been paid by us before your appointment.

  11. Where patients are in breach of agreed payment terms, we may disclose this information to debt collection agencies and legal proceedings may follow. This may result in your name and address being entered into the Computer Bureau default listing which will have an impact on your credit rating.

  12. Costs incurred to recover outstanding monies will be charged to the customer.

  13. Variations to the Terms of Trade may occur from time to time, and Ashburton Health First will notify the patient by way of invoice – receipt of which shall be deemed to be acceptance by the patient.
